Please do not get a band.  That torture device nearly destroyed me.  Maybe you could pre cert it like a DS, and only do the first part...aka the sleeve.

I have this exact insurance. My employer covered bariatric surgery. Met all the requirements Aetna asked for and was approved the first time around. Just follow everything exactly as they have laid it out in their bulletin and you should be fine. 

Definitely call Aetna and get all the info pertinent to your policy. Bariatric surgery doesn't necessarily come standard with the package an employer purchases, in some cases they cover it for your employer group at an extra cost and not all employers opt in. Aetna as an organization covers it but that doesn't guarantee your employer paid for that coverage.
