My friend had VSG May 9 and is in hospital SICK.....SICK....SICK
Here is my honest, 16 years worth of medicine....opinion:
You tell your friend or her family, to tell the SOB GI doc that unless he is willing to FIRST DO NO HARM (that includes physical, mental or emotional harm) then get the hell out of my room and never return. Ask for a second GI consult. Move her to BAYLOR DALLAS and get help ASAP. I can not stand it when we allow medical providers to treat us poorly. It has been my expereince that when you confront the passive aggressive behvavior head on that it will stop.
I recommend something along the lines of
"DR. X, you have made it painfully obvious you disagree with and question my decision to go to Mexico for surgery. I am not here seeking your approval or acceptance. The reality of the situtaion is that I am having a complex medical issue and you are the one consulted to help me in this matter. If you feel you are unable to meet my needs, or feel you are unable to refrain from negative insulting remarks...I am respectfully requesting a referral to another physcian that is more capable of meeting my needs. It is also my expectation that you will continue to provide me with highest standards of care until such arrangements have been made."
Next step, contact the patient advocate of the hospital and if there is not one...ask to speak to the DON or customer care manager. If that does not work, I suggest contacting the legal department there as well.
I also think it would not hurt to remind this physician that word of mouth is either the BEST or Worst type of advertising and which did her prefer?
Bottom line: the physician is either confronted and he decides to change his tactics, or her "team" changes there. I personally would be pushing for a transfer and then pull the trigger on the scope. My fear is that she has a "subclinical leak" and it could end up costing her life.
Is she showing any signs of sepsis? What is her kidney function?
I will keep her in my prayers. I encourgae her "team" to stand up, ruffle some feathers and get the medical treatment SHE IS PAYING FOR.

Thank you all so much! Dr. Alvarez contacted my friends husband last night and emailed specific recommendations for her care requesting IV hydration and strict NPO for 3-4 days with PPI, Reglan, Zofran until her sleeve swelling goes down....he believes that an initial insult 3 weeks ago...either a stomach virus or just the switch from full liquids to food caused her sleeve to swell and start a vicious cycle of nauseau and vomiting which only worsened and must be stopped. He said he has had this happen in 4 patients and all were resolved completely with this treatment regimen and has not recurred in these patients at all....now the hard part will be getting this doc to listen....we can only hope!

Exactly. And I'd bet dollars to donuts that Dr Alvarez would be more than happy to speak with the ill-mannered piece of **** that calls himself a Dr for that poor patient. I'm sure he couldn't be bothered to waste his time on any patient, especially if he says completely inappropriate things about his 'friend' with cancer, not to mention calling Mexico for God's sake, right? What a ***** I agree with someone above who said he should be taken down a notch or two--but doubt the pig could be. He's too high on himself.

The surgeon that removed her gallbladder last Friday saw her today and at least read Dr. Alvarez's recommendations. He responded that it is easy for Dr. Alvarez to make suggestions from Mexico while all of the US doctors do "the work"....however, she is now on the meds he recommended for at least the next two days and also will be NPO (nothing by mouth) for two days) along with IV fluids and then he plans on feeding her and if she doesn't tolerate it placing a feeding tube. At least this is a start....it isn't the 3-4 days NPO Dr. Alvarez asked for but it is better than nothing...I was also hoping the doctors could communicate and collaborate but I think that would be asking far too much! The US doctors only want to "collaborate" with foreign doctors when they are doing "missionary" work that shows up in the local papers...really appalling and makes me sick but unfortunately that seems to be the biased way of thinking among some of our physicians
