Surgeans in NY/NJ/CT/PA/MD? Compiling a list

I am compiling a list of surgeans in the NY/NJ/CT/PA area to send them information on the upcoming national seminar. Could you please give me the name of your surgean and the phone number and/or address? If you see your doctor already on the list, no need to add it.
