Congrats on your surgery!  As for what to take to the hospital...not much.  You will need slippers and a robe if you have one for walking the halls (if you don't have a robs...get an extra hospital gown and put it on backwards to cover with), some loose clothes to ride home in, maybe some chapstick and your own pillow and maybe an ipod.  If you are a makeup person...you might want to take the basics but you probably won't feel like doing too much.  Don't forget the basic toiltries, too.  As for at home, check with your surgeons office for their protocol but basically it is fairly standard...just may have one or two things added or subtracted.  Some form of protein to drink, SF popsicles, SF jellp, SF pudding, low sodium broths and your vitamins that they want you on. Maybe some crystal light packs...but only if you want them.  Plain water is best for you.  Good luck!
