your doctor should be able to tell you about weight loss programs. Not sure where you live, but here in Houston, the Methodist hosp has a 3 month program. I am not sure of the actual cost as my ins covered some....I think I paid $65 for each visit.
For the WL documentation, I listed what I tried....WW, Atkins, etc. and had my Dr provide my weights over a period of years to show the ups and downs. My PCP also provided the letter of necessity.
Basically, My Surgeon's office did all the data gathering. I merely attended the classes and provided my PCP phone # to my surgeon's office and his staff did the rest.
Good luck!!

Which weight loss surgery have you decided on?? Have you researched all of your options? That would be FIRST on the list.
As far as weight loss programs, some insurance will accept Weigh****chers. You get a stamp each time you go to a meeting and that proves you've attended, etc.... Might look into something like that. Sure would be less expensive than a hospital program.
Best of luck on your journey. Please start out by gathering as much information as you can on ALL the WLS procedures!
