PCP Letter

Does anyone have a template of a PCP letter that I could utilize?  I am from Pflugerville, TX, and was declined by UHC last year.  I am not giving up, and want to try again this year, and would like to see if a PCP letter might help.

Thanks Paul.  I do know why I was declined.  It was the fact that I was unable to provide proof that I had been obese for the past five consecutive years.  I have been obese however since I was very young (middleschool/junior high).  UHC just is making me jump thru hoops, and I will do whatever it takes to get my life back.  I just haven't been going to the doctor a regularly up unitl the past few years.  That is b/c now I have the health issues such as type II, depression, joint pain, and torn miniscus in both knees.  This year, I am determined to get this done.  Thank you for your kind words of encouragement.

Ask you insurance just exactly what they will accept as proof of obesity. I know a lady who provided family Christmas photos for the last 5-6 years. It was accepted as proof.  Just a thought.

I dodge the camera wherever they are, but I am sure I can dig something up.  This is great advise!
