GOT A CALL FOR CONSULTATION

So happy!!! Igot the call for a consultation with Dr Dyer on OCT 9 I know that seems far away but he only takes one or two medicare cases a month if an earlier opening comes up I will take it whew lots of work they have my weight loss records and my PCP letter also .. I am on my way!! (Ihope) they are sending me some kind of packet to fill out and bring with at my appointment hm i wonder what all that is???? anyone know?
