Just a little worried..about my list ad evaluation:help:

On this Monday I go to my Female doctor to get a list of the diet she has placed me on. The problem is it is a new doctor *****placed my old one. I only have one diet plan she wrote out to me personlly last year. With this new doctor, How do I know if my diet history is in her notes. And is she suppose to include the diet pills that were written to me too? I called Dr Cantor in Johnston City for the psyche evaluation. We never got to speak to each other. He does not have a assitant to answer the phone and he would call so early that I was out exercising. I called and left a message on his voice service for the 5th of this month and at 2:00. I asked that he called me to let me know if the date was fine and so I could give him my insurance info. too and to get the street address to where his office is. It has been about two weeks and I lefted two-three calls on voice service. Asking him to please call and comfirm the date. He never did. I do not know what to think now. Should I call him and say because I have not got a confirm on the date or you street number.I am force to choose another doctor who could do my evaluation. Thank you for your time. Or should I just keep on trying to get a hold on him through his phone number voice service? This is a little worrisome for me.
