Welcome to the board.  This is a great place to learn more about surgery!  My insurance had an ironclad exclusion written so I had no recourse.  If you think Tenncare should take you, by all means, appeal their decision.  Also, when you get on SS, Medicare will kick in one of these days.  Medicare does cover this surgery.  For me, I took out a loan at http://www.capitalonehealthcarefinance.com/ .  They allow you to borrow up to $25,000.  Now, a year later, the monthy loan payment is less than my copays were for prescriptions!  So, I am really not out any money and it will be paid off in 2 more years!  Hope this helps!
