I need some help from those in M/boro

I lived in middle tn for 5.5 years til my divorce in 2005 and IM trying to locate the dr's i saw when I lived there and get my med records for my WLS per required by ins. Ive located them all except for the one that did my hernia repair in 2004 and I cant for the life of me remember his name. I remember the location, roughly anyhow. All I can do is desribe how I used to get there and maybe someone will recognize what Im talking about and know the building or street name or something. I used to live in Smtihville/Liberty area and would come down the highway 96 and once I came into m/boro i passed a Krystal Burger, then some apartments, then came to a red light and turn right...I think I went thru 2 more lights and then I turned left and there was a large gray dr office on my right, and some one level brown/brick offices on my left and my dr was in that row of offices...

If this makes ANY sense and some one can help me I thank you from the bottom of my heart, however if I just made no sense at all I fully understand lol. I wish I could remember his name! I just remember he was really nice, african american...and it seems there was also a radioligist in that row of offices...

I am not from M/boro but was wondering if you have gotten your hospital records from your hernia repair? The doctor should be on your records and the hospital should be able to give you his office location and phone number. Just thought this may be of some help. Good luck!!!
