Anyone have UHC River Valley ins?

I have UHC and have recently been approved for surgery. They want 5 years medical history to show proof of having been morbidly obese for a period of time. I was too heavy for my doctor to weigh. He wrote a letter to the insurance company explaining why I didn't have weights on my medical records and assured them that I met that requirement. I needed 6 months supervised weight loss with my doctor. There were a few things, those were the main ones. Early on I called the insurance company and they faxed me a list of their requirements. They were pretty easy to work with and I called them often. The only thing I would warn you about. Be sure that you have the referrals. My surgeons office sent me to a sleep clinic that was not in UHC's network. Also, UHC does not require a psych eval unless you have a history of psych problems. My surgeon does require a psych eval, so that has been the toughest thing to get approved.
