BCBS of Tn PPO Question
Anyone have this? What all do they want as far as documentation. Tina is ready to get started so I"m trying to help her with some of the leg work. I think we may have problems because she really has no documentation she throw alot away when we moved here. Plus she really did,nt go to the doctor alot after she turned 18 because no insurance. So if anyone has any ideas or can help we would be so Thankful!!!!!!!!!!!

If you go to BC/BS's web site (located on your provider card) you can download all the forms. Some you fill in what you've done on your own ie. Slim Fast, South Beach, Atkins etc.. Also any Doctor supervised or prescribed med's and/or plans. this should get you started. I assume you already know your plan covers WLS and is not exempt. If not call the number on the back of the card follow the prompts and the a customer service rep. will check it out for you. I've found BC/BS of TN to be extremely helpful after I found out WLS was part of my plan I asked if they would send me a letter stating such and they did had it in about three days. I have BC/BS of TN PPO. I ended up just needing the letter from my PCP and the report from the nutritionist. But I have a very high BMI (over 60) and my doctor had records of my attempts to lose weight.
I got approved in less than 30 days. But it seems my husband's employer's plan is an excellent one.
