I have a problem or a godsend..

Friends, I have this problem and I do not know how to address the issue. My Pcp's office called me today. They said that I would have to write up my own referral letter and give it back to them and they would type it and the doctor would sign it and faxed it over to my WLS office for them to send my paperwork over to my insurance company. The problem is I do not know where to began or even thought of what to say. I have never written an important letter like this before. I am at a lose for works. I thought about coping what my female doctor's referral letter said. But the problem too is I want it to look well thought out, an orignal letter by my doctor. What to do? On the plus side I could made myself look anyway I want too. But the truth is I want the actual to god honest truth written about me from my doctor. I really do not know what to think. If any of you my friends can help me out with suggestions, please do. I am open to all suggestions. Your friend, Tina

Morning Tina, You could just write it this way: Tina Cannon is morbid obese with multi co-morbilities. It is my opinion that it is Medically Necessary that she have the gastric bypass. Sincerely, Dr. Name. You can list in the letter what co-morbilities you have. But make sure it says Medically Necessary. That is important. My Dr.'s letter was a paraghraph, if you want it done professionally, send me an email listing your co-morbilities, weight, bmi, and when I get home Tuesday evening, I will type out you a letter that you can print out and submit it. I do have a degree in Secretarial Science. I also use the tools for spell check and punctuation. Let me know. Love ya, Kathy
