Be sure to check for exclusions.  You can save a lot of hassles if you find out before you get too far.  Some companies like Wal-Mart have BCBS but the company excludes any type of WLS.

Well I went to a seminar at MUSC in charleston. They give you a bunch of forms to fill out. Which pretty much contain all the requirements. I had to Speak with the doctor, get all my medical records from the family doctor. And there is a manditory phy evaluation before they will even submitt it. They make you do all this before you even submitt it because they know the ins will possibley ask.  Depending on where your friend is all she or he has to do is go to one of the set classes or seminars the local hospital has they give you a big packet and it goes from there. Its out of your hands literally. I signed up for the seminar got contacted and they got all my records and appts arranged for me.  I do know that it helps to have a medical issue with it. Like mine being knee pain, which I had been to the family doctor for several times. All she would do was say, lose weight, lose weight. So I am sure that helped in the records. I had proof of past gym member ships , ect.  Oh , ya swelling of the ankles is anther good one. Surely if they are over weight they have that.

We sincerely regret to inform you that you have officially become an "adrenlin" junkie...  And it's a permanent condition that must be treated as much and as often as possible...  May God bless & keep you...
