I was told to put in mostly facts. That they don't care about how you feel. I just put in all my problems, My diets, and how the surgery would help the problems. I hope that helps. Keep me posted.

Deborah, I would include your complete diet history (dates, weights, small description, etc), a full list of your co-morbidities, your families co-morbidities, write a letter stating how the obesity coupled with the co-morbidites affect your everyday life, show them you are knowledgable about WLS and your true desire for it. If you do a search online, you should be able to find ample information to back you up and some examples of insurance letters. I hope this helps. Keep us updated! Leigh Ann

Deborah, as a patient of Dr. Givens, I never had to povide/write an insurance letter. Dr. Givens office took care of that. I was surprised at how little information I even had to provide the surgeon's office. Donna
