Does anyone have an example of an appeal letter I can read to get ideas on how to appeal to my insurance company? I really want to get this denial reversed so I can have this surgery!! Thanks for your help!

Tena, What were the grounds or reason for the denial as stated by the insurance company? If the insurance company stated that the surgery was not medically necessary, then have your surgeon & primary care physician submit a letter along with ofc notes detailing your history of treatment for wt loss, obesity related health problems. If the insurance company denied your request for surgery as a "non covered benefit," then I suggest that you contact your employer's CEO and benefits manager to request that they contact the insurance company to approve for coverage of the surgery. If the insurance company denied for "not a covered benefit," then your employer asked the insurance company not to cover services related to the surgical treatment of obesity. For this reason, you cannot blame, or for that matter, appeal to the insurance company. It is just following the contractual guidelines as set forth by your employer. Good Luck to you...........
