What are the general requirements for insurance to approve a revision? I had VSG in 2014, lost 95 lbs and started regaining last year with no change to my diet and exercise routine. I have gained back around 20 lbs, so not a complete failure but I never got to my goal weight either and am still obese. My bri is 34 now, was 42 when I had surgery.
