Help with Letter of AMedical Necessity

I see my PCP Friday and she gave me a hard time 2 wks ago about a letter of medical necessity.
I was thinking of preparing a letter and asking her to sign it.
Can anyone give me some ideas as what is should say?

Hi Ginny,

I can send you a copy of my letter of necessity that I typed, my PCP read it and signed it for me when I had RNY in 2004.  Send me your personal email address and I will forward to you.  You can cut and paste what you like out of it.
