NC Medicaid Revision

Does anyone know if medicaid requires a certain amount of weight loss in order to be approved for a revision?  I am on a doctor supervised diet for 6 months, I have to do this for medicaid to consider a revision.  Does anyone know if they require a certain amount of weight loss in order to approve?  I am not losing that well, but have been following the doctor's diet pretty well all but the exercise.  I suffer from severe back and neck problems so exercise is very limited.  I have been swimming when the weather permits.
