Help with Revision letter to Insuranced

I have to write a letter to my insuance to get approval for my gastric-bypass revision.

 Who is the insurance comp ??  When I wrote mine - I stuck mostly to the facts and threw in some personal info about my struggles.  I wished I had saved a copy .
