Does Insurance normally cover a revision?

I have United Healthcare they payed for mine in full. But I did have staple line disruption and an ulcer. I know a lot of people that had it paid for, but there has to be a good reason. Usually either a bmi>35 or failure of the first surgery

I have United Healthcare and was denied coverage for a revision.  I had a VBG i***** and want to revise to an RNY.  They denied me stating that my Company had added new verbage to the policy that stated only 1 revision per lifetime would be covered.  (I work for Continental Airlines)  She told me that a lot of the insurance companies were starting to add this new wording.  I am appealing the denial based on the fact that I started this whole process in the beginning of 2007 so I am hoping they will grandfather the policy.  Also, I have a BMI of 46 and co-mordibities.  Should have an answer on my appeal any day now.
