Did anyone have trouble getting approved because, of lack of weight loss history?

I had a weight loss history but I don't know that any of that was told to the insurance company. I did not have any of it documented. I had very little loss during my 6 months of supervised...because I had to becareful not to loss to much or I wouldn't qualify. Mine went thru without problems.

I just had to supply hand written history of weight loss, nothing was documented by Dr from my history ect. My insurance started January 1st of this year, I completed necessary appointments middle of month, approved by 1st week of February and had surgery March 2nd. I know some different insurance companies have high requirements, but some, like mine just need to see the BMI and co-morbidities. Good luck!

My insurance states that it needs documented attempts at weight loss. When I went for my psych eval, the doctor asked me to list all my previous attempts, fad diets, etc. It was sufficient for my insurance. (Anthem BCBS)

Same here. I had a pysch evacuation and the doctor asked me the same questions. But, this 6 month supervised diet helps me so much.
