Did anyone make out some sort of living ill in the event something went wrong during the surgery?

Just  wondering and if so how did you go about it

I already had one and gave my husband proxy to make medical decisions on my behalf.   On 9/11/01 I was rushed in for an emergency hysterectomy and needed 11 units of blood.  I was in really bad shape plus I had cancer.  I had gotten the papers from the hospital about 2 weeks prior and I filled out who I wanted in charge and any special instructions.  I was very clear with my husband what I wanted and did not want.  The papers needed to be signed by witnesses and I had my father sign them.

I wanted to take the burden off my husband should he be put in a position to decide what I would want.   I also wanted to make sure my family knew what my wishes were so no one could argue with him or make him feel guilty.  

I hated filling out those papers, felt very morbin but I'm glad that we did.  We keep a copy in our safe deposit box and one at home.  My husband filled out papers at the same time as I did.

Hello.  My surgeon asks this of all his patients.  I didn't have one, but was able to find "fill-in-the blank" style forms on-line.  In the state of RI, you only have to have two witnesses sign them vs. other states that like for a notary to sign off.
