Insurance requirement question

OK so I have a 3rd party approver through SHPS for BCBS... SHPS appoves it then BCBS apporves it automatically... SHPS requirements are more stringent...

So anyway, SHPS requires me to write a statement of my weight loss goals.  Anyone else have to do anything similar? What did you write??

I have 3 months left of my 6 month diet then everything gets sent to SHPS.  The lady from SHPS told me they "might" accept 4 months so I am going to see if Dr. Royes office will try sending it in at 4 months.

Sorry, I didn't have to write a statement to my insurance company...maybe someone else here did tho and can help you, I'm afraid I don't have much to offer on this one.  Good luck with the rest of your game plan.  I hope it works out for you.

I didn't have to write one - but I would think you would want to focus on things like - being at a healthy BMI, and resolving any other co-morbities you might have - you might even want to talk about if you don't have any co-morbities right now that you want to avoid them as you age.  Good luck!

I tend to agree with Danielle, I think you should also be able to ask them at Dr Ryders office. I'm sure they've had people to write them before and might be able to give you ideas on what to add.

I didn't have to do a statement of my goals to my insurance but before I started going to Dr.Giovanni I contacted Dr. Ryder and she sent me a packet with a bunch of questions. One of the questions was regarding what goals I have for myself after wls. I pretty much listed out things that I have wanted to do but haven't done because my weight has been holding me back. I listed things like swimming with sharks, having children, hiking through the Brazilian rainforest. Just talk about things you want to do but your weight is holding you back.

I didn't have to do this either. But what a great idea! If nothing else, it reinforces your beliefs/reasons for making the WLS decision.

I did this list for my WLS Angel (Laura, LB) when I started this journey. I look at it from time-to-time and am amazed at how many goals I'm already accomplishing. Especially my health goals!

I hope they are able to get you approved at four months! The only 'snag' I had with my insurance was they required me to have a thyroid blood test to make sure I wasn't having a problem with hypothyroidism (which, as you probably know, causes some people to be over weight).

I did not have to do this- but check the insurance board- they may have some great tips for you!  Fingers crossed!!!

I agree with the others on this.  I would put in the letter things like how to be healthy and reduce the risk of co-morbidites ( if you do not already have them).  And also what you want to do when your weight comes off that you can't now that mean alot to you (traveling on a plane, amusement parks with kids, being more active with the family, etc).  But also, they may be looking to see how serious you understand this surgery is ( not an "easy way out").  How it is a "tool" to losing weight, yet you still doing all the work.  That kind of thing.  And also how this IS FOR LIFE and not a quick fix.  Hope that helps!   Good luck and let us know how it goes.    Eldy
