Medical records

Anyone know if we have to bring our medical records for the consult?  When I made the appt. the secretary said no but how do they determine if you are eligible.

You don't have to bring any. If you like, you can bring a weight loss chart of your past 5 yrs. So they can send it to Ins.   What they do is weight you. They use  your Weight and Height to determine your Body Mass Index (BMI.) Does this help?

One of the requirements of my doctor was a letter from my doctor saying that I was in good health for the surgery and that they support and recommend the surgery.  The records that they want have to come from your doctor.  You don't need to bring them.

I didn't bring anything.  You should have answered all their questions when you filled out the questionaire on-line before making your appointment.  If anything is needed he will let you know.

Hi Fogo,

I brought test that I had already had done recently that I knew where going to be need for pre-op test.  I figured why do them again if I just had them done.
