Hello did anybody have to do a registration form on line for ri hosp. And how long before you hear from them? Someday hope to have lapband. Thank-you lisa benevides

Do you mean for University Surgical Associates?? They have an online form to fill out...after you submit it, wait about a week and then call them...they will have a nurse call you to go over the forms and qualify you for an appt. I had Dr. Roye for the lap band from there...LOVE HIM...love my band!!! Good luck and if you have any questions, ask away!! Kristen D
