Hi Mary, I would like to welcome you here. There is a great bunch of people here. I had my surgery May 26 and so far have lost a total of 106 lbs some of that before surgery, So if you have any questions I am sure someone can help you. Lots of luck to you. Claudia

hi there! unfortunately, i started this whole process long before Univ. Surgical Assoc. started using that questionairre - so i never had to do it. [it took me a lot longer than most people because i had major insurance battles, almost self paid, but then got a new insurance that paid for it right at the last minute!] Is there anyone else out there who can help Cheryl? I'd imagine someone here must have the info. If no one answers you soon - just call Annette at Dr. Roye's office 553-8310.

thanks Kristen, I called Dr Roye's office today and got the correct web site. It took a good 30 mins to fill everything out. Hope I hear from them soon. Thanks again, Cheryl

Hi Cheryl, I just filled out my questionaire for Dr.Harrington at the same web site last week, and you copied it right. https://www.usasurg.remedymd.com/index.php you have to create a account and I think they have some sort of support site that you can pay for but you don't have to do this to fill out the questionaire. Hope this helps good luck, I am still waiting to hear back from them. I think I might have to give them another call. Michelle
