How do I work up the courage to take action?

I can very much relate to where you're coming from.  I was fat my entire life, tried diets too numerous to mention, lost weight gained it back (plus some) more times than I care to count.  

Unlike you, I waited until my late 40s to do something about it, and suffered (needlessly) and deprived myself of life's treasures for too many years.

What region do you live in?  I ask because it might be helpful for you to attend one of the Barix pre-op meetings  (2nd Tuesday evening from 7 - 9 and First saturdays from 11 - 1:30) ... they are for people who are CONSIDERING surgery as well as those who've already decided but haven't yet had it.  You would get the "real deal" information from people who've been through it (some recently, up to some who are years out from it) and can tell you much more about the experience and what to be prepared for than you'll ever get from an article or questionnaire.  They'll also be able to tell you more about how to find a doctor that you're comfortable with, because that selection is one that will be a very important one for the rest of your life.

Your nervousness is completely normal - we've ALL been through it.  The thing to focus on is WHY you're considering this step and what you expect to achieve as a result.  The surgery is most definitely not "taking the easy way out."  Nor is it a fix-all.  You will have to work hard at to achieve the maximum effect, and you will have to address the underlying reasons for your weight gain in order to maintain your weight loss.  Having said that, it is 1000% worth doing and has given me a new life and new strength in so many ways.
