My MD required that I take a minimum of 4 weeks off post op. I have a desk job, too, but I appreciated having the extra time off to heal and get used to the new eating patterns, etc.
After 3 weeks I was defnitely ready to go back to work, though.
I took short term disability, which covered 60% of salary, then I supplemented my PTO for the other 40%, so I got a full paycheck the entire time out (just short on PTO for the rest of the year).

Like the others said, fax your paperwork to Barix, and then definitely call them and make sure they received it. As for the time off, I took off exactly two weeks, and really could have gone back after one. At the time of my surgery I was a webmaster, so pretty much just on my butt all day writing code, unless I had to get up and walk to a meeting. I actually felt better when I went back to work -- sitting around all day at home alone is NOT for me.

Congratulations! It took me a loooooooooong time to get approval and I made the nurse keep saying it over and over! Can you get to any pre op groups? They could answer a lot of questions for you.
If you could schedule your surgery during your Christmas break you may not have to take any time off of school. I was ready to go back after about 3 weeks even though I was told 4-6. But since I teach at a University I waited until the end of the Spring semester and had all summer to recoup.
