Revision Q for Eastern PA: BCBS, Keystone, Independence, Amerihealth, ICHP

Has anybody had a revision in PA? What insurance do you have/did you have?
I am trying to fight an Independence BCBS denial.
Independence Blue Cross (IBX) is the largest health care provider in Eastern PA and has these affiliates and subsidiaries which share the same criteria (and probably deciders):
* AmeriHealth® provides cost-effective managed care products in a number of markets.
* AmeriHealth Administrators® provides benefits management services for health plans nationwide.
* Keystone Mercy Health Plan is a Pennsylvania Medical Assistance (Medicaid) managed care health plan.
* Keystone Health Plan East
* Independence Administrators
* QCC Insurance Company
* Vista Health Plan, Inc.
* Independence Blue Cross and Highmark Blue Shield Caring Foundation
I am miserable with my current surgery outcome; can you help me?
