Member ID #

Has anyone on here recieved an email from OH with a Photo and Testimonial Consent Form attached.  I was told to sign it and fax it or scan it/email it back to OH.  They also asked for my Member ID# which is supposed to be under my Name in my profile???   Just curious if this is real or why they sent it to me.  I just added some new pics and changed my Avatar...hmmmmm
