Not losing weight!
Brian, I cannot help you with the Kaiser part. But if that is their requirement, I would do everything I possibly could to comply with their requirements so I could have the surgery. It is so very much worth the effort you have to put into it.
If you have a hard time with traditional diets, try something like Atkins. It's a lot more liberal with portions and certain types of foods, and you have less hunger. If you can drop the weight you are required to drop using Atkins or something like that, then you can just focus on doing the diet to maintain your loss while you wait for surgery.
Good luck!

Brian - I just read your profile and saw the info about the Depakote for the bipolar 1. I was erroneously diagnosed with bipolar 2 (I have ADD but am hard to diagnose) and for a short time was put on a half dose of Abilify. I told the doc I would not take anything that made me gain weight. Well, I have to say that not only did I lose my appetite, I just was completely disinterested in food or eating while on it. Twice in one week I completely forgot to eat for the day. Granted, I am post-op, but for me that is not usual. You might see if your doc thinks Abilify might be a good sub for the Depakote. It's actually a med for schizophrenia I guess, but they are finding it works wonders for bipolars. That might help you get a good foothold on some weight loss until you can have the surgery.
Good luck!
