Insurance not approving wls?

I HAVE BEEN WORKING TOWARDS THE SURGERY FOR MOST OF YEAR 2003. FINALLY, I GOT EVERYTHING IN ORDER AS THE DOCTOR REQUIRED. AFTER THE DOCTOR SUBMITTED MY PAPERS TO INSURANCE COMPANY FOR PRE-AUTH, THEY RECIEVED A LETTER BACK SAYING THAT THIS SURGERY IS NO LONGER COVERED AS OF JAN 2004! THEY HAD ALREADY INFORMED MY DOCTOR OFFICE AS LATE AS NOVEMBER THAT I MAY BE COVERED UNDER MY PLAN! WHAT TO DO? (REGENCE BLUE CROSS, BLUE SHIELD, PPO)
