Your best bet is to contact Walter Lindstrom right from the very first denial.

I hope you get some help with an appeal--I really think it is an insurance game they play with us to weed out the people who aren't really serious about the surgery--or it gives them time so they don't have to pay. Whatever--I know how frustrating it can be! Keep trying!
