The NP will advise you as to WLS requirements at your initial appointment with her.  In my case, I had to go thru OHSU's requirements in order to have surgery.  The requirements are as follows: - Initial appointment with NP - Nutritional counseling with dietician - Cardiac clearance from cardiologist - Sleep study - Psychological evaluation Once all the requirements are completed and met to OHSU's satisfaction and insurance is authorized/approved, the surgery date is set up. After the surgery date is set up: - Surgery prep seminar - Pre-admit tests at the Pre-Admit Test clinic - Consultation with assigned surgeon - Surgery! Hope this helps!

I have United healthcare and they have different requirements for different versions of United Healthcare. Get in to your first visit with the nurse practitioner. While I was in my appt. the insurance person made the call to my insurance and created a check list of what I had to do. I think there are both insurance requirements and office requirements. The NP will fill you in and you can ask at that time which is which. Get in as soon as you can to your primary care physician and the bariatrics office. Tell them about the 3 month requirement so each visit can count towards this!

Tina, I have already jumped thru all the hoops and gone thru a number of obstacle courses and am scheduled for surgery tomorrow.  The response should be directed to the OP (circus). Thank you.
