Example of appeal letters

Hi, quite a while back I stumbled upon a thread here in the forum which said it was examples of appeal letters (at least I think it was a thread). I'm having trouble locating it and I finally got my denial from my insurance (my employer is self-insured though so there's hope still) so I need to write an appeal letter and thought it would be good if I could locate the example letter agian to give me some ideas.
