state employees question

Those of you who are Oregon State employees did you fill out your OFLA/FMLA forms?  and if so, which appointment/doctor did you take it to have it filled out? I am going to be pushing it on having enough leave for my surgery.  The way it stands right now I'll only have about 3 1/2 weeks of leave.  So I need to not use anything in the meantime.  I have short-term, so I'm okay to be in LWOP.  BUT   I have a supervisor who hates LWOP and she doesn't seem to be very supportive of the fact that I may need 6 weeks off.  Plus, I work for the Justice Dept and they are real sticklers when it comes to fillout out OFLA/FMLA forms for EVERYTHING even if you aren't going to be in LWOP.   So, who did you take your forms to?  the first consult?  the surgeon?   Thanks in advance.

I did fill out the FMLA forms, particularly because I was in a situation like you.  I didn't have a lot of sick leave available and, just in case I had a complication, I wanted the protection to return to my job if I had to be off longer than anticipated.  I actually faxed the form to the NP just a few days before surgery and had her sign it.  That seemed to be fine. See, that's the other reason why I want to do it.  Just in case there are complicaitons.  I mean, I have an ideal of being out 3-4 weeks.  But some people need longer.  and, just in case, I'd like to still have my job!

Yes, I thought I'd need only a couple of weeks off, but I'm just not young anymore -- 53 -- so I needed to take three.  I'm a pretty tough old bird, but this surgery can knock the stuffing out of you (hey!  that's pretty funny! ). Linda

Which surgery are you having?  With the band, my husband took a week and that was perfect.  He's a state employee.  He used sick time, are you using all that? :) Melanie

I'm having the gastric bypass.  Yes, I'll be using my sick leave -- but our department requires you to use your OFLA/FMLA pretty much all the time.  If you're sick, you get asked questions about it and get sent the papers.  It's a pretty big deal for us.  So, regardless of if I use it all or not... I'll have to fill out my forms.

I had to fill out my OFLA/FMLA form for my surgery because I had a few complications and almost ran out of sick time.  I thought that I was going to be off only two weeks but it was longer than that but no matter what happened I am glad I had the surgery and my bosses weren't that upset with me about the time off because I am healthier than I have been in many years.
