Kaiser Questionnaire!! 18 Pages :)

I received my Kaiser NW's Questionnaire in the mail today. They were not kidding when they said it would be a thick packet  lol. 18 pages of questions I needed to fill out. For those wondering the 18 pages of questions were broken into 8 sections: Questions about; Medical history, Habits and Daily Life, Weight and Weight Loss History, Eating Habits and Attitudes, Current Family Support, Family Background and Experiences, Current Emotional Health, and lastly Current Physical Health and Functioning.  I will mail it back tomorrow and hopefully I will get into the July class if it is not already full?? But if it is full or it is to late to get into that class I know my information will be there in enough time to get me in the Aug. group class.  So far Kaiser is on the ball. Only took 11 days (Including the weekend & The fact they only mail out once a week) to get the Questionnaire I was thinking 14 days or more.

Good to hear that you are on your way to making progress.  I am not with Kaiser, but just mailed in my information packet to Legacy.....hoping to hear from them in the near future so I can officially begin the process.  I am eager to have the surgery.  This waiting seems like forever, but in reality, I suppose it is good to wait. Good things come to those that weight. (pun intended)

I hear you both,yes it did take a long time for me to. I think I went to the orientation in Jan and I just had my surg on the 18th. Also ,there are more people getting in line for the surgery. Jo Ann

Good luck getting in by July. I filled out my questionaire with Kaiser and it took two months to get the class because they were full. The best thing to do to help speed up the process is enroll in the Freedom from Diets class as soon as you can. Kaiser only has them every so often and I had to wait 4 months to get into mine. You don't have to go to the orientation first to enroll in the Freedom from Diets class.

blay****36 Thanks for the info. I have heard Kaiser does take a while and since you just went through it I am glad you gave me your input!! As for your advice to sign up for the class I just did that. I don't have to do the freedom from diets because I was told from Mary Lou the case manager that I live too far and that the 5wk weight management would be okay. I also signed up for the Kaiser barriatric support group that meets locally for me. I enjoyed that and recommend it to all who are starting the process or who have already had WLS. I am also glad you told me that I may not get into the July class or even the Aug. one Thanks for the heads up. I am trying to not get anxious :) but that is not working :)

It sounds like you are on the ball. You are lucky to not have to take the 12 week ( but I took the 5 week one a long time ago) and thought the 12 week one was very benifical just it took a while. I lived in Longview until about a year ago right before I enrolled in the program. Stay up on Kaiser and try not to get to excited as it will make your journey seem even longer and believe me with Kaiser it will be longer.

Just got a call back from Kaiser and yep group meetings are all full until Sept.
