Hi Kathleen. Thanks for the heads up about 'clear definition of medically surpervised weight loss program'. I called my ins. company and they swore to me that every thing stated on the coverage guidelines was what had to be done. No loopholes. I did find out however, that the mental health evaluation they want has to be done within the six month time period too. Nola
