surgery date!

I will be having surgery September 22nd , make room on that bench for me :D

referred mid Jan, info session feb 29th, nurse April 25th, nut class may 3rd, Doctor may 10th, social worker may 12th, dietician & sw July 11th, Doctor July 19, surgical education class August 22, meet the surgeon August 31, surgery September 22/16

congratulations what hospital? i will be going for hernia repair surgery sept 23 at St joes in Toronto with Dr Sullivan. 

Referral Feb 14 2014; Orientation July 30 2014; 1 on 1's Feb 17, 2015; Pre op May 27 2015; Surgery date June 16, 2015
