2 months away from surgery and have some timeline questions

Im in the home stretch and was just wondering what happens from here. Maybe it's nerve etc but I just don't want to mess this up its been a long road to get here. Last time I spoke with anyone was in July to get my surgery date

My surgery is Monday March 14 at HRRH with Dr Starr

When do I get called in for pre screening and Opti ?

When do I tell my employer and my health care provider Great West Life ?

Who fills in my forms for STD ? Family Dr or Dr Starr ? Who writes my letter for work ?

Do I bring my Std forms to be completed when I do the pre screening? fax to them now ?

When will they call or should I have called them ?

I know I'm rambling , again don't want to muff this up
