I am new to the forum and the process. I was referred by my Doctor on May 28th, after months of researching and now being 7 months smoke free. I went for orientation in Guelph on June 11th and have my first appointment with the Nurse on Wed the 17th. So excited to get a second chance!

Referral Date: May 28/15 Orientation: June 11/15 1st apt. June 17/15 Social worker & Dietician 1st apt. July 27/15 2nd apt. Social Worker, Nurse, & Dietician Sept. 21/15

Welcome. I went through Guelph and found them to be kind and supportive.

CENTURY CLUB MEMBER at 6 months post-op.

Referral to Guelph Feb/13, Sleep study and all bloodwork and ultrasound May/13, orientation July/13. Nurse, NUT,SW Sept/13, 2nd NUT, nurse and SW, 3rd round and cleared for surgery Dec/13. Pre-op Apr 7/14, Surgeon May 2/14, Opti Jul 3/14, surgery Jul 17/14.
