Hi, sorry if it wasn't too clear. The package you receive with your Questioneer and the Physicians Questioneer. My Dr told me it is not covered by OHIP for him to complete it and charged me $75, I had it all completed and sent it in on Friday so that Humber would have all my information along with a copy of my sleep study and some other lab work before the Orientation.

I sent it by Xpresspost as well so they will have to sign for it so I know it gets there safely and on time. Now the lady at the Humber Clinic says they will not send it to Guelph as Guelph has different paperwork to complete. When I asked her what they will do with it she said they will just destroy it. Really.

Wow my doctor never charged me for filling in any forms. But each doctor does what they want.
