Surgeon and location?

Hi everyone.  Has anyone had a gastric band removed and a bypass performed at the same time?  If so, which Surgeon did you have and what site would it 

be done at??

Just doing the research myself for the same reason.  I have come accross two in Montreal that are self pay.   Dr Michel Gagner and Dr. Christou.  Both have excellent reviews.
