Short Term Disability claim for RNY surgery

Has anyone else put in a short term disability claim with their benefits provider for the time they were off work recuperating from the surgery?  Is there any tips/tricks to ensure this goes smoothly?  My provider is Manulife, if that is of any value to the conversation.

I meet with the doctor tomorrow and have my forms with me, but if there is any special wording that will ensure this goes smoother, I would appreciate it.

I was put on STD and I am also with Manulife.  My doctor filled out paperwork my employer had and there was no special wording.  I had no issues at all.

I made a short term disability claim through my employer, although it turned out that I had enough sick days to have a month off and only used one disability day on the claim. I had a form to bring to my surgeon to fill out, it was simple form that asked for dates and if I would be able to resume all duties on my return. I brought the forms in a week before surgery and she didn't fill out the forms until my fourth week appointment! Luckily my employer was ok with that and I still received full pay but I'm not sure everyone would be as lucky. I wasn't impressed.

I kept mine simple, GI Surgery, OHIP covered and did not have an issue. Good Luck !

My provider is also Manulife and the Dr filled out the form after surgery and I submitted it to my work when I left the hospital. I informed my work before hand as to the date I'd be off, but my sick forms have to handed in within a week so my pay is not interupted. Good luck, my work only started covering it this year, they used to consider it cosmetic surgery!!!! Lame

I'm with Manulife as well, all I needed was a letter from the clinic and was approved for 4 weeks.  They said if any longer then would require paperwork filled out by doctor

I was on STD for 2 months recovering.  I had no problem at all.  I just filled in the blanks, there were no tricks or tips to worry about.

I also had my STD through Manulife. It was no trouble at all. Get your forms signed and sent in, and there's nothing else to do. I had no issues getting my 6 weeks (pretty standard) and the rep I dealt with was very nice. I was also a bit nervous as my surgeon put 4 week on the forms, and at the end of the 4th week I was still exhausted all the time and was worried they'd send me back to work and I wouldn't be able to do it. But, as it turned out, no issue getting the additional 2 weeks, the last of which was gradual return.

just remember that while you are off on STD, that if you belong to a Pension plan, you will not be contributing and therefore may have a "bill" at the end of your term/year.
