I know a few women that have had their surgery with Dr. Klein, and they say he was great! I personally don't have him, but from what I've heard you are in good hands :) 
As for the tests, I believe it depends on the patient. I did not have a U/S or an upper GI, or any of these things. I did have a sleep study, and this is done most of the time with patients. I believe those tests are done depending on what Dr. Glazer (internist) decides. He will be someone you meet after you have met with your surgeon, nurse, dietitian, and social worker. 

The time frame honestly depends all on what tests you need, etc. I personally will have waited 10 months, from the time of my family Dr's referral to the time of surgery. Also I've had some cancellation appointments along the way, always ask to be put on a cancellation list. My surgery date was due to a cancellation and so were my RN/RD/SW appointments.
