A friend is going through the approval process to OHIP.  Is there anything in particular you need to make sure you tick off on the application or say YES to - to be SURE you get approved.  Seems to me I remember something like that when  got approved.

I just had my RNY on the 2nd and I did not have to be approved by ohip. I told my gp that I wanted the surgery and he sent in the referral to the Bariatric Network.  I am not sure why she needs approval, haven't heard of it in Ontario. 

 Hw: 301 Day of surgery: 287

Are you referring to the old OCC form?  That hasn't been used since 2009. 

The doctor applies to the bariatric registry now.

Im having my RNY done on April 22nd and I never had to do anything either.

There is criteria for the surgery but the doctor fills out the form as mentioned above.
