Depends on where you are located.. 

I had my PS in Scarborough with Dr Nandagopal. 

I had to have documentation of rashes and what I used to help with the rashes (it was a cream) 

The panni must hang a minimum of 10cms; the dr should measure your belly and if you want your breasts done, they will measure them too. 

You have to be at your goal weight or very close to it. Also you must maintain this weight for a minimum of one year. When I had seen my PS, he told me he wanted me to lose 10lbs more and maintain that weight for at least a year. I complied with his request.

The surgeon should take photos, he/she will write a report and submit everything to ohip and they will decide within 8weeks. 

With my surgeon, at that point, I went in again and chatted more about the surgery and I had to pay for the upgrades.
