1st referral 2011, Second referral 2012, Orientation Kingston Nov. 2012, SW, NUT, DR. Dec. 2012, Sleep Test Jan. 2013,Nut Feb 2013, CPAP Machine Started Mar. 2013, Pre-Op Class April 2013, Second Sleep Test April 2013, Meet with Dr. Hagen April 2013, PATTS HRRH April 25th 2013, Opti Fast April 30th 2013, Dr. Glazer May 7th 2013, Surgery May 21st 2013, YAY!!
