Northern Ontario Travel Grant Question

My referring physician is on holidays this week and I forgot to pick up my travel grant. I leave for TWh at 6:55am Tuesday and since it's a long weekend her office won't be open. My question is, can I send the travel grant to TWH when I get back home and have them fill their part out?

yes you can. when you go for your follow up appointments bring them with you and they will send them down to get a signature.  may take some time but we are all used to waiting right?  good luck to you and we will be moving on down the bench for you

Heidi - is this ur first trip to TWH ??  You can go online and fill it out - print it - leave it at TWH for signature they mail it back to you - then you can take it to ur doctor for signature and forwarding to the Ministry of Health Office

What is a travel grant?

It's a a form the a referring doctor fills out and you bring it with you when you so to see another doctor out of your area ... it's only available for people in Northern Ontario.

my question is are they getting approved?  Here in Thunder Bay we were told at the orientation we don't qualify for the Northern Travel Grant if going for wls....unless this has changed???

I am in Thunder Bay and was not told this, and was even given the information package from the bariatric clinic in regards to applying for the travel grant

Mine was approved when I met with Dr Klein in Toronto last april.  the grant is for medical procedures that aren't perfomed in the hospital in your area so WLS is definately covered as they don't do them here in Thunder Bay
